To obtain a prescription refill, please call the Fort Worth Heart pharmacy line at: 817-338-4725. Please be prepared to provide the following information when requesting a prescription refill:

Patient’s Name
Patient’s Date of Birth
Medication Name, Strength, and Dosage
Pharmacy Name and Phone Number

Please note that some refills may require the patient to first be seen by a physician.
